Are you an experienced and proactive Social Worker passionate about supporting adults through hospital discharge? Leeds City Council is seeking a dedicated Adults Hospital Discharge Social Worker to join their fast-paced hospital team, ensuring timely, safe, and person-centred transitions from hospital to home or care settings.

Job Responsibility:

  • Carry out Care Act assessments and Discharge to Assess (D2A) planning
  • Coordinate care packages, placements, or reablement services
  • Liaise with NHS staff, community services, and families to create safe discharge plans
  • Complete Mental Capacity Assessments and safeguard vulnerable adults
  • Maintain accurate and timely case records

Requirements:

  • Qualified Social Worker with Social Work England registration
  • Experience in hospital discharge, adult social care, or multi-agency work
  • In-depth knowledge of the Care Act, MCA, and safeguarding practices
  • Ability to work well under pressure in a fast-moving environment
  • Strong assessment, coordination, and communication skills
