Job Description

You'll be based within the hospital discharge team, working closely with health professionals, care providers, families, and community partners. Your role is key in facilitating safe discharges, preventing delayed transfers of care, and ensuring individuals receive the right support at the right time.

Job Responsibility

  • Carry out Care Act assessments and Discharge to Assess (D2A) planning
  • Coordinate care packages, placements, or reablement services
  • Liaise with NHS staff, community services, and families to create safe discharge plans
  • Complete Mental Capacity Assessments and safeguard vulnerable adults
  • Maintain accurate and timely case records

Requirements

  • Qualified Social Worker with Social Work England registration
  • Experience in hospital discharge, adult social care, or multi-agency work
  • In-depth knowledge of the Care Act, MCA, and safeguarding practices
  • Ability to work well under pressure in a fast-moving environment
  • Strong assessment, coordination, and communication skills
