Job Description:

Adult qualified social worker position in Bexley, Greater London. Responsibilities include conducting assessments, developing care plans, safeguarding vulnerable adults, and collaborating with teams and families.

Job Responsibility:

Conduct thorough assessments of adults' needs in line with the Care Act 2014 and relevant legislation
Develop and implement individualized care plans to foster independence and support community integration
Safeguard vulnerable adults by providing targeted interventions to ensure their safety and well-being
Work collaboratively with multidisciplinary teams, families, and community services to provide comprehensive support
Advocate for adults' rights, ensuring access to necessary resources, services, and benefits
Regularly review and adjust care plans to respond to evolving needs and situations
Uphold statutory responsibilities and follow best practices in safeguarding and care provision

Requirements:

2 years experience post qualification
Action safeguarding procedures
