Job Description:

As a Complex Care Worker in this hybrid role, you will support adults with complex needs, working 2 to 3 days per week in Bexley (Civic Offices) and engaging with the local community. Your duties include comprehensive assessments, personalised care plans, and multi-agency collaboration, with a focus on person-centred care.

Job Responsibility:

Conduct comprehensive assessments of individuals' needs in line with the Care Act 2014
Develop and implement person-centred care plans, ensuring statutory compliance under the Care Act 2014
Collaborate with multiple agencies to ensure integrated support, following the Mental Capacity Act 2005 and Care Act 2014
Participate in Court of Protection work, including attending hearings if required, in accordance with the Mental Capacity Act 2005
Identify and address safeguarding concerns, ensuring protection under the Care Act 2014 and Mental Health Act 1983

Requirements:

Relevant social work qualification
3 to 4 years post-qualification experience (including ASYE) and supervision experience
knowledge of Court of Protection work and attendance when required
