Job Description:

Hammersmith & Fulham Council is seeking a dedicated Adult Independent Living Worker to support residents in regaining or maintaining independence, accessing outcome-focused support, and navigating the care system seamlessly. You will work collaboratively with residents, carers, professional staff, and partner agencies to co-produce creative solutions that meet assessed needs, promote choice, and support independent living.

Job Responsibility:

  • Assess, plan, and review support for adults using strength-based, person-centred approaches
  • Assist residents with mobility aids, home adaptations, and assistive technology to maintain independence
  • Collaborate with multi-disciplinary teams across community and hospital settings
  • Monitor risks, contribute to safeguarding processes, and provide advice to colleagues
  • Maintain accurate records and data in line with statutory and council guidelines

Requirements:

  • NVQ Level 3 (or equivalent) in Health & Social Care or relevant field
  • Knowledge of the Care Act 2014, Mental Capacity (Amendment) Act 2019, and safeguarding procedures
  • Experience in strength-based practice, person-centred approaches, and independent living support
  • Excellent communication, problem-solving, and organisational skills
  • Competent IT skills, including electronic case management systems (e.g., Mosaic)
  • Enhanced DBS check required
