Admissions Registered Nurse Home Care

• Completes Admissions (Starts of Care, Resumptions of Care, 2nd Service Evaluations)
• Completes routine visits as indicated
• Evaluates, plans and administers medically prescribed nursing treatment to patients in their home
• Performs a comprehensive physical, psycho-social, and safety assessment of assigned patients
• Completes thorough documentation of Admissions, Resumptions of Care, and Second Service Evaluations
• Plans for and provides skilled nursing treatment in the home reassessing patient status on each visit
• Coordinates patient care with other clinical team members
• Completes Agency Recertifications
• Discharge planning occurs throughout the episode of care
• Demonstrates proficiency and competency in the agency/program automated clinical record
• Fosters a “Culture of Safety”
• Maintains up to date schedule in computerized system
• Maintains and enhances individual knowledge as well as promoting professional standards in nursing

• Minimum of 2 years of prior home care experience
• Proficiency in OASIS Assessments and documentation
• Comfort managing a variety of clinical and psychosocial needs
• Organizational and communication skills
• Flexibility
• Previous experience with EPIC preferred
• Bachelor of Science in Nursing
• Massachusetts Registered Nurse License
