Job Responsibility
Assist in implementing effective community transport services, including producing rosters and transport schedules
Conduct regular reviews and update service plans in consultation with clients and their representatives to ensure ongoing individual needs are met
Provide vital feedback to the Team Leader regarding client issues, requests, concerns, and compliments
Maintain strict client confidentiality and ensure meticulous documentation within the management system to meet statutory and reporting requirements
Manage and maintain organisational records in accordance with strict policy procedures and practices
Requirements
Experience in Home Support services and/or Aged Care is essential for this role
Willingness to Drive vehicle/transport Clients as required. You must hold a Current C Class licence
Working with Children Check – or Vulnerable Person-Related Employment Screening check required
