Job Description:

This role is responsible for supporting patients who have a substance/alcohol use disorder to navigate the health system to obtain services to help them in their recovery pathway. The incumbent, who identifies as a person with substance/alcohol use disorder, will be part of a collaborative multidisciplinary team in the Center for Recovery Medicine (CRM) providing a unique skill set and peer support. The incumbent will assist the patient to access medical services, community services/resources and develop healthy behaviors by building a trusting relationship with the patient and providing specific support related to the individuals recovery pathway. They will engage with patients to determine individual recovery goals and develop a recovery plan. Engagement may be initiated in the inpatient setting, outpatient clinics (MAT, Perinatal) or during an emergency room visit. Engagement is maintained depending on the patient’s preference through ongoing telephonic/video support and/or accompanying the individual to appointments (medical, support groups). The incumbent must be able to educate, advocate and be knowledgeable of harm reduction practices, multiple recovery pathways, and community resources to be able to help the patient meet their recovery goals. Incumbent will document interactions with patient in EPIC Medical Record timely and effectively. Individual will attend regular Community of Practice meetings to share experiences with other team members and engage productively and effectively with all members of the CRM team on behalf of the patient.

Job Responsibility:

  • Provide care coordination including outreach, referrals, access, MATP, Buprenorphine provider, follow up appointments
  • Engage patients in a clinic or hospital setting and establish a rapport as evidenced by empathetic and active listening, communicating in a non-judgmental way, and utilizing harm reduction approach
  • Determine individuals recovery goals and develop a recovery plan with the patient
  • Provide patient with resources to share with patient to support recovery plan
  • Coordinate with the multidisciplinary team to assist individuals in their recovery journey by linking them to recovery services
  • Provide advocacy for the patient with other team members, i.e.. nurses, social workers
  • Document each patient encounter in detail according to protocols and guidelines
  • Follow up by telephone with patients who have been discharged

Requirements:

  • High School/GED
  • 1-3 years of experience working with individuals in recovery, which may include non-paid work such as facilitating recovery support groups or acting as a sponsor for individuals in recovery
  • 1-3 years of experience living in, volunteering in, and/or working in high risk communities
  • 1-3 years of knowledge of local community resources (housing, food, support groups)
  • 0-1 years of ability to collaborate with clinical professionals, medical providers and other members of the care team
  • 1-3 years of recognize signs and symptoms of addiction with sensitivity to cultural and environmental differences
  • Certified Recovery Specialist OR Certified Family Recovery Specialist with in 1 year from the date of joining
  • Knowledge of Harm Reduction Approach
  • Active listening skills
  • Social perceptiveness, interpersonal, organizational skills
  • Ability to utilize a computer for entering data
  • Act 34 Criminal Background Clearance Certificate
  • Act 33 Child Abuse Clearance Certificate
  • Act 73 FBI Fingerprinting Criminal Background Clearance Certificate

  • Participate in multidisciplinary treatment team meetings and assist in development of the treatment plan
  • Communicate treatment interventions and support needs for assigned caseload to applicable interdisciplinary team members and to support systems as needed
  • Offer training support to other direct care team members less familiar with cognitive behavioral treatment interventions (MHT, nurses, clinic assistants, etc.)
  • Providing parent or support person training and education on how to support CBT interventions and behavioral treatment plans effectively. This may involve teaching positive reinforcement techniques, modeling appropriate behaviors, and providing strategies for managing challenging situations
