Current and valid HCPC registration
Experience working within acute hospital settings, ideally in acute/general medicine
Excellent communication, clinical reasoning, and organisational skills
The ability to work autonomously and within a fast-paced MDT environment
A strong focus on safe discharge planning and patient-centered care

Job Responsibility

Provide specialist assessment and intervention for patients with acute medical conditions
Manage a varied caseload, supporting timely and effective discharge planning
Collaborate with the multidisciplinary team (MDT) to ensure a holistic approach to patient care
Implement rehabilitation programmes and functional assessments to support patient independence
Supervise and mentor junior staff and contribute to service development initiatives

Requirements

HCPC registration and relevant professional memberships
Previous experience in an acute hospital setting, ideally at Band 6 or Band 7 level
Strong knowledge of discharge planning, frailty, orthopaedics, neurology, and general medical conditions
Excellent communication and interpersonal skills to work effectively within a multidisciplinary team
A proactive and patient-focused approach to care delivery

Job Responsibility

Conducting comprehensive assessments of patients with a range of medical, surgical, and neurological conditions
Developing and implementing individualised treatment plans to support patients’ rehabilitation and discharge planning
Collaborating with multidisciplinary teams, including physiotherapists, nurses, and medical staff, to ensure a seamless patient journey
Providing specialist advice on adaptive equipment, functional independence, and environmental modifications
Supporting patients in early mobilisation, cognitive rehabilitation, and activities of daily living (ADLs)
Managing a caseload of complex cases and prioritising care effectively in a fast-paced environment
Contributing to service development initiatives and mentoring junior staff or students where applicable
