Job Description:

Atrium Health Navicent Medical Group is looking for an Acute Care Nurse Practitioner to join our General and Bariatric Surgery practice.

Job Responsibility:

  • Mix of Acute Care Surgery
  • Provides assessment, diagnosis, treatment, education, referral, and follow-up for a shared or owned panel of patients or may handle on a by-visit basis
  • Recommends screenings and tests based on patients' demographics
  • Manages health care within protocols mutually agreed upon by physician and APP team members
  • Identifies abnormal findings early and initiates appropriate intervention
  • Participates on committees and in research as appropriate
  • Facilitate administrative and clinical activities under the supervision of the Sponsoring Physician
  • Collaborates with other advance practice providers, nursing and ancillary staff and community agencies to provide continuity of care for patients
  • Ensures patient safety and confidentiality

Requirements:

  • Must be a graduate of an accredited Nurse Practitioner program with a Master Degree in Nursing
  • Must maintain a current RN and APRN license to practice in the State of Georgia
  • Must demonstrate competence in their practice setting and must be able to perform with a high degree of accuracy to reduce errors in judgment related to care of patients
  • Must obtain required CEU's in care of patients in the care setting each renewal period
  • Must maintain a high degree of confidentiality
  • Must be familiar with a variety of general surgical procedures

  • Collaborate with Transitional Care Managers, home health, social workers, hospitals, SNFs, and specialists
  • Structure and prioritize scheduling based on clinical complexity for both new and routine patients
  • Recognize, diagnose, and manage both acute and chronic medical conditions in order to prevent destabilization and readmissions
  • Provide after-hours and weekend call support, shared with other in-state providers
  • Facilitate and conduct goals of care and advance care planning discussions with patients and families
  • Assess and evaluate family/caregiver needs and limitations
  • Teach patients, caregivers, and others about their health conditions
