Job Description:

The Activity Coordinator is responsible for planning and implementing a therapeutic activity program for the individual patient while on the Transitional Care Unit (TCU). He/she works closely with the interdisciplinary team to meet the physical, mental and spiritual needs of the patient.

Job Responsibility:

  • Planning and implementing a therapeutic activity program for the individual patient while on the Transitional Care Unit (TCU)
  • Working closely with the interdisciplinary team to meet the physical, mental and spiritual needs of the patient

Requirements:

  • Must have knowledge of Title XXII, OBRA regulations, and MDS
  • Successfully completed a course on Activity Coordinator within 90 days of hire/transfer
  • Two years experience in social or recreational program within the past five (5) years, one year of which was full time in patient activities program in a health care setting or have completed appropriate training in a course designed specifically for this position and approved by the State
  • Minimum of at least 1 year of activity experience in long term care or transitional care
  • Excellent customer service skills required
  • BLS required

  • Support site visits, including coordination, scheduling, and catering
  • Plan and assist with travel arrangements for the leadership team
