They are responsible for managing and reconciling medicines following patient hospital discharges, a critical task for ensuring continuity of care and preventing errors. Furthermore, these professionals are the go-to experts for addressing medication-related queries from both patients and clinical staff, managing drug shortages and recalls, and leading on medication synchronisation initiatives to simplify dosage schedules for patients.
