They play a crucial role in managing patients with long-term conditions (LTCs) such as diabetes, hypertension, asthma, and cardiovascular disease. By working in accordance with national clinical guidelines, they help optimise treatment plans, adjust medications, and support patients in managing their health effectively. A common and highly valued responsibility is acting as an Independent Prescriber, which allows them to assess, diagnose, and prescribe treatment for patients within their competence, thereby increasing patient access to care and alleviating the workload of GPs. The core responsibilities of these roles are deeply clinical and collaborative. Typical duties include conducting structured clinical assessments, managing minor illness clinics, reconciling medications following hospital discharge to prevent errors, and providing expert advice on complex drug interactions and polypharmacy. They work autonomously while being a key member of the multidisciplinary team (MDT), collaborating closely with GPs, practice nurses, nurse practitioners, and other allied health professionals to ensure seamless, joined-up care for every patient. To succeed in GP Practice-Based Pharmacist jobs, specific qualifications and skills are typically required. Candidates must be registered pharmacists with the General Pharmaceutical Council (GPhC).
